Privacy Policy
This Privacy Policy explains how Caladrius Therapy, PLLC (“Caladrius Therapy,” “we,” “us,” or “our”) collects, uses, and shares information when you visit caladriustherapy.com (the “Site”), contact us through the Site, or interact with our online booking tools.
Scope of this policy
This policy covers information collected through our website. Protected Health Information (“PHI”) created or received in the course of treatment is governed by the Health Insurance Portability and Accountability Act (“HIPAA”) and described separately in our Notice of Privacy Practices. If you become a client, the Notice of Privacy Practices governs how we handle your clinical records; this Privacy Policy governs the website itself.
Information you provide to us
We collect information you choose to share with us through the Site, including:
- Contact and consultation forms. When you use a form on the Site (for example, the consultation form on the contact page), we collect your name, email address, and any message you choose to send. These form submissions are processed by our website host, Netlify, and delivered to our intake team.
- Email and phone contact. If you email us at hello@caladriustherapy.com or call us at 704-980-3082, we receive whatever information you choose to share.
- Therapist match quiz. Answers entered into the “Find Your Therapist” quiz on /match are processed in your browser only and are not transmitted to us unless you separately contact us with the results.
- Booking and client portal. If you schedule an appointment through the “Book Appointment” links, you are taken to our practice management vendor, SimplePractice. Information you enter in the SimplePractice widget or portal is collected directly by SimplePractice as our HIPAA business associate, not by this website. See SimplePractice’s privacy policy for details.
Please do not include sensitive health details, diagnoses, or other Protected Health Information in website forms or unencrypted email. Our website forms and standard email are not secure channels appropriate for PHI.
Information collected automatically
Like most websites, we and our service providers collect limited technical information automatically when you visit the Site, including:
- IP address, approximate location derived from IP, device type, operating system, and browser type and version.
- Pages viewed, referring URL, links clicked, time spent on pages, and similar usage data.
- Date and time of access and other standard server log information.
Cookies and analytics
We use cookies and similar technologies to operate the Site and to understand how visitors use it. Specifically:
- Google Analytics 4. We use Google Analytics to measure aggregate website traffic and conversion events (for example, form submissions and booking-widget opens). Google Analytics may set cookies and process IP addresses on our behalf. We do not send Google Analytics any name, email, or other directly identifying information from our forms. You can opt out of Google Analytics by installing the Google Analytics Opt-out Browser Add-on or by blocking cookies in your browser. For more information, see Google’s privacy policy.
- SimplePractice booking widget. Pages that load the SimplePractice booking script may set cookies controlled by SimplePractice for the operation of the booking widget.
- Embedded maps and fonts. The contact page embeds a Google Map, and the Site loads typefaces from Google Fonts. Loading these resources causes your browser to make a request to Google, which may include your IP address.
Most browsers let you refuse or delete cookies through their settings. We do not currently respond to “Do Not Track” signals because there is no consistent industry standard for them, but you can use the controls described above to limit tracking.
How we use information
We use the information described above to:
- Respond to your inquiries, schedule consultations, and provide customer support.
- Operate, maintain, secure, and improve the Site and our services.
- Measure and analyze how visitors find and use the Site, including which pages and calls to action perform best.
- Comply with legal obligations, enforce our terms, and protect the rights and safety of Caladrius Therapy, our clients, and the public.
How we share information
We do not sell or rent personal information collected through the Site. We share information only as follows:
- Service providers. We use a small set of vendors to operate the Site and our practice, including Netlify (website hosting and form delivery), Google (analytics, fonts, maps), Contentful (content management for non-personal website copy), and SimplePractice (scheduling and electronic health records, under a HIPAA business associate agreement). These vendors process information only as needed to provide their services to us.
- Legal and safety reasons. We may disclose information if we believe in good faith that it is required by law, subpoena, or court order, or necessary to investigate fraud, protect the safety of any person, or enforce our terms.
- Business transfers. If Caladrius Therapy is involved in a merger, acquisition, sale of assets, or similar transaction, information may be transferred as part of that transaction, subject to this policy.
Data retention
We retain website inquiry messages for as long as reasonably necessary to respond to you and maintain business records, after which they are deleted or archived in accordance with our internal retention practices. Analytics data is retained according to the default retention period configured in Google Analytics. Clinical records are retained according to North Carolina law and our Notice of Privacy Practices, not under this Site policy.
Security
We use reasonable administrative, technical, and physical safeguards to protect information collected through the Site, including HTTPS encryption in transit and access controls on accounts that receive form submissions. No method of transmission over the internet is 100% secure, however, and we cannot guarantee absolute security. As noted above, please do not send PHI through website forms or unencrypted email; use the secure SimplePractice portal or call us instead.
Your choices and rights
You have the following choices regarding information you share through the Site:
- Access, correction, deletion. You can ask us what website inquiry information we hold about you, ask us to correct it, or ask us to delete it by emailing hello@caladriustherapy.com. Requests to access or correct clinical records are handled under HIPAA and our Notice of Privacy Practices, not this policy.
- Opt out of analytics. Use the Google Analytics opt-out add-on or your browser’s cookie controls (see “Cookies and analytics” above).
- Email. If you contact us by email and later wish to be removed from any reply thread, simply let us know.
Some U.S. states (including California, Colorado, Connecticut, Virginia, and others) provide their residents with additional rights regarding their personal information, such as the right to know what categories of information are collected, the right to request deletion, the right to correct inaccurate information, and the right not to be discriminated against for exercising these rights. Caladrius Therapy does not sell personal information and does not use it for cross-context behavioral advertising. To exercise any state-law right that applies to you, contact us using the information at the bottom of this policy.
Children’s privacy
The Site is intended for adults (typically a parent or guardian seeking care for themselves, a partner, or a minor in their family). We do not knowingly collect personal information directly from children under 13 through the Site. If you believe a child under 13 has submitted information to us, please contact us and we will delete it.
Links to other websites
The Site links to third-party websites and services, including SimplePractice, Google Maps, and our social media profiles. We are not responsible for the privacy practices of those third parties; their use of your information is governed by their own privacy policies.
Crisis information
Website forms and email are not monitored 24/7 and are not appropriate for emergencies. If you are in crisis or concerned about your safety or someone else’s safety, call or text 988 (the Suicide and Crisis Lifeline), call 911, or go to your nearest emergency room.
Changes to this policy
We may update this Privacy Policy from time to time. When we do, we will post the revised version on this page and update the “Last updated” date below. If the changes are material, we will take reasonable steps to notify you, such as a notice on the Site.
